[bug-gs] 8.12: a number of minor issues

Mouse,
A single bug report with all of the issues is fine since if we want
to open up separate bugs in the bugs.ghostscript.com bugzilla system,
we can easily.
If it is convenient to send the documentation issues separately from
the build issues, that's slightly preferable, but currently the
documentation and build issues both end up with the same Artifex
engineer, Ralph Giles, although others pitch in occasionally.
