The referenced PostScript file contains Type 3 fonts and was generated 
by latex.  After conversion to PDF by Ghostcript, some glyphs are missing.

Ghostscript aborts on page 21 due to an out of range error, but Acrobat 
keeps going.  Alex has submitted a patch.
