Attached TIFF file of the result using the cidfmap supplied in comment #2 using
the font supplied in comment #3 and the test file in comment #1 (with the
addition of a showpage so the result is visible on the display).

This appears to work correctly on HEAD for me using Windows. NB also used the
command line switch '-I/gs/Resource/Init' in order to correctly find the
cidfmap file of course.
