Read the manual - if you want letter size all the way, here is how it is done.

pdftops -paper letter ...
pstops -pletter ...
ps2pdf -sPAPERSIZE=letter ...

only ps2pdf is part of ghostscript.

(pdftops is part of libpoppler; pstops is part of psutils, neither is part of
ghostscript, but anyway, documentation for either are available)
