I have a similar problem with GS HEAD:
when trying to convert the attached Japanese PDF into PDF with GS and pdfwrite,
i get a gswin32c.exe application error ( Windows 7 x64 ). When converting it to
tiff (tiffg3 device) everything works fine.

Here is my cidfmap
/MS-Gothic << /SubfontID 0 /CSI [(Japan1) 3] /Path
(c:/Windows/Fonts//msgothic.ttc) /FileType /TrueType >> ;
/MS-PGothic << /SubfontID 1 /CSI [(Japan1) 3] /Path
(c:/Windows/Fonts//msgothic.ttc) /FileType /TrueType >> ;
