It is conventional to do so to clarify the intention, and to make 
clear the licensing even when the source is copied outside the original 
package.

is safest to attach them to the start of each source file to most 
effectively convey the exclusion of warranty; and each file should have 
at least the "copyright" line and a pointer to where the full notice is 
found."

A more lightweight version is just to state that the code is LGPL
in the copyright header.
