The first page of CUPS raster is stored left-to-right, top-to-bottom. The BMP
has bottom-to-top pixel order for positive heights and top-to-bottom for
negative heights (read from wikipedia
http://en.wikipedia.org/wiki/BMP_file_format). Maybe bmptopnm cannot deal with
negative heights or you tried to process second page (back-side for duplex
print), which might have different pixel order according to CUPS spec.
